What We Do
We provide skilled nursing care, restorative therapy and medical social services to patients in their homes or wherever they may reside, including assisted living facilities. Under the direction of our healthcare professionals, we administer medical services to the patient while strictly adhering to the physician’s plan of care. Our services create a continuum of care throughout the disease process. We emphasize patient and family education, which empowers the patient to take an active role in the management of their condition. This is supported by our Caregiver Support Education classes conducted at the initial care stage to all members of the family.
